CME COURSE: Massive Transfusion Protocols: Current Practices and New Innovations for Managing Traumatic Hemorrhage

Massive transfusion practices have changed considerably in the last several years from primarily laboratory-guided therapy to formula-driven protocols. In this program, the rationale for developing massive transfusion protocols will be reviewed. The evidence in the literature supporting and refuting the use of high ratios of plasma to red blood cells will also be discussed. Lastly, new ideas and products that should be considered for use in a massive transfusion protocol will be described. There are no participant prerequisites for this activity. The result of the PROPPR study was published in JAMA in 2015 (JAMA 2015; 313(5): 471-482). There were no significant differences in the primary outcomes of 24-hour and 30-day all-cause mortality between the 1:1:1 group and the 1:1:2 group. Death due to exsanguination was decreased in the 1:1:1 group and more patients in that group achieved hemostasis. No other safety differences between the two groups was noted.

TARGET AUDIENCE: This activity has been designed for general, specialty, and subspecialty physicians who prescribe blood products, care for blood donors or have related interests. There are no participant prerequisites for this activity.

At the conclusion of this course, participants should be able to:

  1. Define a Massive Transfusion and describe the important elements required in creating a massive transfusion protocol (MTP).
  2. Discuss the proven and controversial aspects of blood product ratios in formula-driven MTPs. Describe the advantages and challenges in using Group A plasma for trauma resuscitation.
  3. Identify 'new' blood products such as whole blood and cold-stored platelets that may be beneficial for use in trauma resuscitation.
