CEU COURSE: SEROLOGICAL TECHNIQUES USED IN ANTIBODY IDENTIFICATION

Multiple techniques useful in assisting in antibody identification beyond the initial panel results and resolving ABO discrepancies are presented. These techniques include the use of: enhancement reagents, prewarming, neutralization, saline replacement, enzyme treatment and antibody titration.

In this program, common reagents and methods used in a reference laboratory to identify and confirm antibodies detected in the initial panel results will be presented. The basic reagents and techniques will be arbitrarily grouped into the following categories for ease of discussion: investigation of ABO discrepancies, enhancement reagents, serum techniques, cell techniques, elutions and titrations. However, as black and white as these categories appear, antibody identification is often quite gray. Many times, techniques from several of these categories will be used simultaneously on the same serum sample; and many times, the results from the use of one of these techniques will trigger the use of another.

LEARNING OBJECTIVES
At the conclusion of this course, participants should be able to:

  1. List the techniques that can be used to resolve ABO discrepancies.
  2. List and describe three enhancement reagents that may be used in routine antibody detection and identification.
  3. Describe the three prewarmed techniques that may be used to characterize a cold autoantibody and/or exclude underlying alloantibodies.
  4. State the principle of the neutralization technique.
  5. Define the saline replacement technique and give an example of when one might need to use it.
  6. Describe the use of enzyme treatment in antibody identification.
  7. State the principle of elution methods.
  8. Describe the antibody titration technique and define the following terms: titer, endpoint, and score.
